- Copy of Parent’s Driver’s License/ SSN.
- DHF Immunizations Records (Form 680).
- DHF Physical Form (Form 3040).
- Report Card or Transcript from previous school.
- Verification of Domicile.
- Verification of Guardianship. (if applicable)
- Scholarship Award Letter (Scholarship required documentation).
- IEP, which is learning disability. (if applicable)
